likely to receive follow-up care within a week for an urgent medical condition<br />
than those with Medicaid or without health insurance.<br />
    A new study shows that callers claiming to have private health insur<span id="more-77"></span>ance<br />
were about twice as likely to get a follow-up appointment at an ambulatory<br />
clinic within a week after an emergency room visit than those without insurance<br />
or with Medicaid.<br />
    The results suggest that callers&#8217; health insurance status may be more<br />
important to most people answering the phones and scheduling appointments at<br />
ambulatory care clinics than their health status.<br />
    Researchers say that while 98% of the clinics contacted screened callers for<br />
health insurance status, only 28% attempted to determine the severity of the<br />
callers&#8217; medical condition.<br />
    Health Insurance Makes a Difference<br />
    Researchers say Americans will make about 114 million visits to hospital<br />
emergency departments in 2005 and more than 80% will be treated and discharged<br />
with recommendations for follow-up care. But many uninsured people often<br />
experience problems making follow-up appointments within the recommended time<br />
frame.<br />
    In the study, published in The Journal of the American Medical<br />
Association, researchers looked at the role insurance status plays in<br />
securing a follow-up appointment for an urgent medical condition identified<br />
during an emergency room visit.<br />
    Research assistants made calls to nearly 500 ambulatory clinics in nine U.S.<br />
cities from May 2002 to February 2003 and identified themselves as new patients<br />
who had been treated in an emergency room and required an urgent follow-up<br />
appointment within a week. Callers read a script detailing one of three<br />
possible medical scenarios requiring follow-up, including pneumonia, high blood<br />
pressure, or a possible ectopic pregnancy (a potentially life-threatening,<br />
early pregnancy occurring outside the uterus, such as within the fallopian<br />
tube).<br />
    The same assistant called each clinic twice with the same medical scenario<br />
but with a different insurance status.<br />
    Insured vs. Noninsured: Getting an Appointment<br />
    The results showed that the availability of follow-up appointments varied<br />
according to insurance status. For example:</p>
<p>      64% of callers with <a href="http://www.carinsuranceeasily.com/">private insurance</a> were offered a follow-up appointment<br />
within a week vs. 47% of callers overall.<br />
      Callers with private health insurance were more likely to receive<br />
appointments than those claiming to have Medicaid coverage (64% vs. 34%)<br />
      Callers with private health insurance also had higher appointment rates<br />
than those without insurance who offered to pay $20 and arrange for payment of<br />
the balance (65% vs. 25%)</p>
<p>    But researchers found no difference in follow-up appointment rates between<br />
those with private insurance and uninsured people willing to pay cash for the<br />
entire visit fee (about $100 for a typical visit).<br />
    &#8220;These study findings suggest that reported insurance status influences<br />
access to follow-up appointments for patients with conditions requiring urgent<br />
ambulatory follow-up care,&#8221; write researcher Bret Asplin, MD, MPH, of the<br />
department of emergency medicine at Regions Hospital and HealthPartners<br />
Research Foundation in St. Paul, Minn., and colleagues. &#8220;Although the<br />
ultimate consequences of these access barriers are not known, they may result<br />
in patients delaying needed follow-up care, risking adverse outcomes, or<br />
requiring additional emergency care or hospitalization.&#8221;</p>

You still have to qualify medically to get health insurance through Nationwide. Nationwide offers some of the most comprehensive health plans available on the market today. Nationwide health plans offer low maximum out of pocket. Some plans that they offer work similar to the way HealthNets plans work. Once you meet your deductible nationwide covers everything at 100% and Nationwide plans cover doctor visit before you meet your deductible and Nationwide is the only health insurance company that has no prescription drug deductible on most of their plans. If you are looking for the most competitive HSA plans, Nationwide will be your choice.</p>
<p>PacifiCare is company that has been available to Californians for a long time until recently they were bought by United Health Care. PacificaCare will be replaced by Golden Rule health plans. If you have PacifiCare you might want to find out if you will have to re-qualify medically for new health insurance once they take the company of the market. Golden Rule owned by United Health Care witch known as the quality company and recommended everywhere. If you are considering PacifiCare I would wait for Golden Rule or get something else.</p>

My Health Insurance of Washington is not qualified to give tax advice, but the next few items should be discussed with<span id="more-73"></span> your tax preparer.<br />
H.S.A. Plans: Health Savings Account qualified Health <a href="http://www.carinsurance-site.com/">Insurance Plans</a> (H.S.A.s) have two components: 1) a qualifying health insurance plan, and 2) a tax advantaged savings account.  May people who have H.S.A. plans never set up and take the full advantage of the tax benefit associated with the allowed savings account component.  Amounts (up to certain limits) contributed to your H.S.A. savings account can be deducted dollar for dollar from your top line income on form 1040 (line 25 on 2008s form).  The money contributed tax free is yours to keep (its not use it or lose it) and to spend on any qualifying health expenses you or your family incur now or in the future. If you know you are going to have health expenses, you might as well pay for them with your tax free dollars.<br />
Self-employment deduction: This is a no brainer for the self-employed or people with a small business. Health insurance premiums for you and your family can in many cases be taken as a business expense. (In 2008 on form 1040 it came out on the first page on line 29.) In addition, if you have an H.S.A. plan, not only can you take the top line deduction from your income for amounts contributed to your H.S.A. account as explained above, but your monthly H.S.A. health insurance premiums can also be taken as this business expense.<br />
Health Expenses greater than 7.5% of your AGI:  If your total health insurance expenses for you and your family are greater than 7.5% of your Adjusted Gross Income, amounts spent OVER that 7.5% can be taken as a deduction. What qualifies? Much more than you think! Please refer to the IRS site http://www.irs.gov/publications/p502/ar02.html for the full list. Many people just dont keep receipts, and dont realize that money spent on a whole host of qualifying health expenses could have been taken as a legitimate deduction. If you had a major health event, had a new baby, or other combined health expenses which could put you over the 7.5% threshold, make sure you look into this.<br />

<p>In the 12 months to June 2008, HBA paid more than $1 billion - or over $2.8 million a day - in benefits to members. This surge in claims represented an increase of more than 10% - or $103.4 million - on the previous financial year.  </p>
<p>HBA&#039;s 10 most expensive health <a href="http://www.cheap-autoinsurancequotes.net/">insurance claims</a> in 2008 totalled $1.1 million, while its highest claim was made by a member who underwent brain surgery - at a cost of $152,000.  </p>

<p>Similarly, the Health Services Index showed costs for hospital and medical services rose by 6.1% in 2008. Yet despite these ever-growing costs, 89% of all medical services provided to HBA members in hospital were gap free in 2008.</p>
<p>&#8220;Irrespective of the fact that improved medical technologies and treatments are escalating the costs of healthcare, we&#039;re determined to keep downward pressure on members&#039; costs,&#8221; Bowden said.</p>
<p>HBA has been able to consistently keep its average private health insurance premium increases below the industry average due to efficient management practices and business operations.  </p>
<p>The average dollar increase for HBA cover is less than $1.50 for singles and $3.30 for families per week - with the Federal Government rebate.</p>
<p>&#8220;All HBA customers can have confidence that we will continue to support their health, and that of their families, via our cost-effective and comprehensive range of products and services,&#8221; Bowden said.</p>
<p>HBA customers will be notified in writing of the changes to their insurance premiums. The changes are effective from Wednesday, 1 April.</p>

The current system forces people to stay in jobs longer than they might want to because they fear losing their health insurance, said Tom Beeman, who became Lancaster General&#8217;s president <span id="more-68"></span>and chief executive officer in June. Beeman previously led a network of hospitals in Nashville, Tenn. Beeman also thinks the system leaves many people who lose their jobs desperate for affordable  coverage. </p>

<p>&#8220;I believe that we have to figure out a mechanism that (allows) everybody to have access to health insurance,&#8221; said Beeman, who also leads Lancaster General&#8217;s network of health-care facilities throughout Lancaster County &#8220;You can do it through a voucher system. We could do it through tax breaks.&#8221; Beeman rejects the idea of a national health care system, saying it would be inefficient.<br />
Beeman, 52, met with the Business journal June 24 in his office at Lancaster General Hospital. He discussed Lancaster General&#8217;s competition with for-profit hospitals, the hospital&#8217;s economicrevitalization efforts and why hospitals should help employers keep workers healthy.<br />
CPBJ: For-profit hospitals are prevalent in the South. Will your experience leading a nonprofit health system in Tennessee help you guide Lancaster General as it competes with two for-profit providers in the county (Lancaster Regional Medical Center and Heart of Lancaster Regional Medical Center)?<br />
Beeman: When I was in Nashville, we had an academic health system, as well as for-profit (hospitals). And I used to say, and I meant this sincerely, that the academic organization was a great competitor because it really pushed me to excellence. The for-profit provider was a good competitor because it pushed me to efficiency. I always thought I had the best of both worlds, and that&#8217;s what I dunk we have as an organization here in Lancaster. We have an extraordinarily highlevel organization that has all the benefits that a big teaching hospital could have &#8230; But we also have the efficiencies that come from understand ing what the community needs and wants &#8230; I hope that my experience in working both collaboratively and competitively with the proprietary hospitals will be useful here.<br />
CPBJ: Lancaster General Hospital is involved in the WellFirst Gold program, an effort to promote workplace wellness in Lancaster County. Should hospitals be more involved in helping companies keep employees healthy?<br />
Beeman: We have an intrinsic responsibility, because of our unique backgrounds in education and health, to help the community stay well. At the end of the day, staying out of hospitals should be everybody&#8217;s goal in health care. Regrettably, it&#8217;s not the reality, because not everybody accepts full responsibility for their health.<br />
CPBJ: &#8220;Personal responsibility&#8221; is a buzz phrase among employers trying to reduce their health-insurance costs. What do you think about forcing employees to become more responsible for their health-care spending?<br />
Beeman: Consumers are willing co-conspirators (contributing to rising health-care spending) because they are consumers. Americans consume health care like they consume other things &#8230; I do think that co-pays, at least at some level, are appropriate because they really make the employee a co-partner with the provider as well as with the company in that. (Note: Beeman also said technology, competition between providers, and administrative burdens from insurers and government contribute to rising costs.)<br />
CPBJ: While at your previous job, you brought a Catholic hospital together with a Baptist one. Do you have any advice for companies trying to bring different cultures together in the workplace?<br />
Beeman: Unless you&#8217;re really upfront and understanding of what the cultures of the organizations are, you won&#8217;t be successful. What (I attempted) to do was not to impose one culture on another, but to help define (the culture we wanted to be and move as a group toward that new culture) &#8230; I think you have to have patience, respect for individuals. And you have to have a clear-cut vision and keep calling people back to that vision, because that&#8217;s the thing that keeps people motivated and keeps moving them.<br />
CPBJ: Your predecessor helped start the James Street Improvement District, an effort to improve Lancasters northwest neighborhood. Will Lancaster General remain committed to economic revitalization?<br />
Beeman: I have a strong belief that this is a vital part of what we&#8217;re about. It&#8217;s about building community, and we&#8217; can&#8217;t serve our broader community if there&#8217;s no downtown Lancaster to be part of. Building up our community with our partners like (Franklin &#038; Marshall College) is vital, and I intend to be fully engaged in that effort.<br />
